1. Were you denied equal opportunity in hiring, up-
     grading, rate of pay, promotion, demotion, trans-
     fer, layoff, discharge, selection for training, or
     other working conditions?
2. Was this because of your race, creed, color, or na-
     tional origin or was it b)ecause you were not fitted
     for the job, or lacked seniority, or for other rea-
     sons?
3. Was anything said or done to make you feel that you
    were discriminated against because of your race,
    creed, color, or national origin?
4. Were you required to answer questions concerning
    your race, creed, color, or national origin either
    orally or on application forms, including supply-
    ing your photograph ?
5. Does the company make it clear in its recruitment
    advertising and through posters that it is an equal
    opportunity employer?
6. Do you believe that company policy or your super-
