OCTOBER 13, 1942.-Committed to the Committee of the Whole House on the
state of the Union and ordered to be printed
Mr. NICHOLS, chairman of the Select Committee to Investigate Air
Accidents, submitted the following
REPORT
[Pursuant to H. Res, 125]
CONCERNING AN ACCIDENT WHICH OCCURRED AT ABOUT 5:37 A. M.,
APRIL 11, 1942, WHEN THE NC 16064, OPERATED AS TRIP 12 OF
UNITED AIR LINES, CRASHED IN FLUSHING BAY DURING AN
ATTEMPTED LANDING AT LA GUARDIA FIELD, NEW YORK CITY'S
AIRPORT, RESULTING IN INJURIES TO THE FIRST OFFICER AND
STEWARDESS
I. CONDUCT OF INVESTIGATION
Executive hearings were conducted by your committee at LaGuardia
Field, New York City, on April 16 and 17, 1942, to ascertain the cause
or causes of the above accident. Others present for the committee
were: Irving G. McCann, counsel; Charles S. Greaves, clerk; and
Jane Young, reporter. Consideration was given to the weather which
prevailed at the time of the accident, the adequacy of United Air
Lines' operations, the mechanical condition of the NC 16064, and the
conduct and judgment of the pilot before and during the let-down and
landing procedure.
The following witnesses testified under oath before the committee:
William S. Tuttle, dispatcher, United Air Lines.
John Scarpulla, airway traffic control operator, LaGuardia Field
control tower.
Girard C. Boon, Jr., junior airway traffic control tower operator,
LaGuardia Field.
Joseph L. Deutsch, meteorologist, United Air Lines.
John Howard Hutchinson, beaching chief, Pan-American Airways.
George Wirshup, seaman, Pan-American.
