CRIMINOLOGICAL REVIEW

VOL. II. PART I. JANUARY, 1934

        PRESIDENTIAL ADDRESS

   SOME MEDICO-LEGAL ASPECTS OF
                       SHOCK*

By SIR BERNARD SPILSBURY, M.B., F.R.C.P.
I HAVE selected this subject because shock may occur in forms
which present to the medical man most difficult problems for
elucidation. In these forms the sudden onset, and, it may be,
sudden death, offer little opportunity of observing the bodily
changes as they occur, and seldom give any indication of the
mechanism by which death or the symptoms were brought about.
Shock I regard as one of the most overworked terms in
the medico-legal vocabulary, as many deaths said to be due
to shock should really be ascribed to such conditions as loss
of blood, exhaustion, injury to a vital organ, concussion of
the brain, and so forth. Cases in which persons can justly
be said to owe their death to shock are comparatively rare.
It is an elusive term to define. Shock may develop gradually,
 appearing many hours after the injury which has caused it;
its most striking manifestation is a progressive fall of blood
pressure to a level far lower than is found in health, and this
may continue to a fatal issue. On the other hand, the fall may
be arrested and recovery ensue. Of this variety of delayed shock
