Age-Determination of Injuries of Internal Organs
Summary. Age-determination of injuries of internal organs is of great practical importance
in forensic medicine. In most instances these investigations are performed on cadaver material.
Environmental conditions for the cadavers and the post mortem interval are different from
one case to another. The use of modern morphological methods, such as histochemical enzyme
methods or autoradiography are therefore impossible for the determination of age of a tissue
injury. Light microscopy must consequently be used for age-determination of injuries. The
various stages of the reparative process must be recognized for this method of evaluation.
Three general reactions are significant in this process: 1. the degenerative changes of the
injured cells, 2. the local reaction of the blood circulation and 3. the reactions of non-injured
parenchyma and connective tissue cells in proximity of the injury. The time relationship of
these reactions can only be studied in experimental animals. The results of such experiments
indicate that time relationship of cellular reactions vary with many conditions. For instance,
delay of cellular repair is observed in animals which are injured before or during starvation
or hemorrhagic shock. The cellular reaction is also diminished in older animals. Results
obtained from animal experiments may only be applied to humans with extreme caution.
The histological parameter of injury to adipose tissue, liver and kidney is demonstrated. The
following parameters are of importance. The presence of necrosis or fatty degeneration of the
damaged cells, the appearance of polymorphnucleated leucocytes, the appearance of iron-
pigment, changes in the size of nuclei and mitoses in the parenchymatous and connective
tissue cells in proximity of the injury indicating proliferation, the appearance of new-formed
mucopolysaccharides, collagen and elastic fibers. Cellular repair reaction is especially depen-
dent on the extent of the injury.
It is impossible to use only one of these histological criteria. As many as possible of these
criteria must be compared and correlated with the conditions under which the injury was
inflicted.
Zusammenjassung. Fur die Altersbestimmung der Verletzungen von inneren Organen
stehen, im Gegensatz zu den ausgedehnt untersuchten Hautverletzungen, heute immer noch
im wesentlichen nur lichtmikroskopisch nachweisbare Parameter zur Verfilgung, da sich eine
Reihe moderner zellbiologischer Methoden fur den Einsatz am menschlichen Leichenmaterial
nicht eignen. Um eine Altersbestimmung von Verletzungen vornehmen zu kinnen, ist die
genaue Kenntnis des zeitlichen Ablaufs der Reparationsvorginge notwendig, der nur in tier-
experimentellen Serienuntersuchungen zu ermitteln ist. Es ist jeweils die Prfung notwendig,
inwieweit dieser experimentell ermittelte Reparationsablauf mit jenen Reparationsvorgangen
zeitlich korrelierbar ist, die durch Untersuchungen von Verletzungen der inneren Organe des
Menschen bekanntgeworden sind. Keinesfalls konnen die tierexperimentell ermittelten zeit-
