KENNEDY ASSASSINATION RECORDS ACT OF 1992 chief, Physical Security Division 29 August 1955 Acting Chief, Physical Survey Branch Security Violation - Lost Classified Material - Found on Guard's Desk in H&s Building - PHILLIPS, David Atlee 1. On 22 Au-ust 1955 at 12:15 P. ., PBS Guard Morris reported that he had found a brown envelope on the gard desk in &S Building which someone had inadvertently left there. The face of the envelope bore the name Dave Phillips and the words Hand Carry. The envelope whic was seald contained various completed personnel forms on Mr. ormaniattervait4 an applicant for Agency employment. One of the -personnel forms was clas ified COMFIDENTIAL. 2. On 22 August 1955 Mr. Ralph E. Tobiassen, of this Branch, investigated the violation. A check of Badge Office files that David Atlee Phillips was assigned to Di/?/PP. Accordingly, Piss [ro . Wolgamott Security Officer, Ds3/P/Pi, was informed of the matter and Mr. Phillips, Secial Assistant to the Chief, PP/ICD/Analysis and Assesseent Branch, was interviewed. 3. Mr. hi1lips advised that he was custodian of the material. He stated that upon leaving R&S Building at about 12:00 Noon on the data in question, he ha1 placed the envelope on the Oward's desk while signing the Guard Register for a visitor and had forgotten to takethe envelope with him. He advised further that he had returned at about 12:15 P. M. to look for the material and was informed that the :uard had turned It over to the Office of security. CCkCLUIO': 4.
