         The true cost of malntainlAg the elderly and sick at
hose has been largely hiddem because the greatest portion of
that cost ropr~sents the value of services provided by families
and friends rather than those provided at public expense. the
poxat 5n the iapairment scale whoa hose service costs . Jma
institutional costs falls at the aid-point of the greatly
impaired group when families and friends are zoviiJ4    about
$237 per month fo every $120 spent by agencier. About 1*1 of
noninstitutionalized older people fall in the aa above the
break-evon point. On the average, it wcld still cost the public
more to institutioaalize these people because agencies spend
less per person for hose care services than for institutional
care. The cost impact of possible changes to the Medicare
program which would increase the avtilability of services and
provide service* not currently coverod by the program would be:
elimination of limits en number of visits under Darts A and
B--S12.5 million; elimination of skilled care requirement under
Parts A and 5--$1.25 billion; elimination of prior
kospitalization  equirement under Part A--$12.5 Million;
elimination of homebound requirement under parts A and 9--$92.5
aillou; and adding homemakr/chore servioes--T5 miion. The
agencies delivering home services have developed differing
criteria for eligibility. duratiom, scope of covered services,
and method of reimbcrsemen*, and because of these v*rying
criteria, effective coordination seems almost Impossible. RS)
