(CGS), of Nashville, Tennessee, under request for proposals (RFP) No. HHSM-500-2014-RFP-0096, issued by the Department of Health and Human Services (HHS), Centers for Medicare and Medicaid Services (CMS), for a Durable Medical Equipment (DME) Medicare Administrative Contractor (MAC).
