          Pursuant to section 801 (a)(2)(A) of title 5, United States Code, this is our report on a
          major rule promulgated by the Department of Health and Human Services, Centers
          for Medicare & Medicaid Services (CMS) entitled Medicare and Medicaid Programs;
          Changes in Provider and Supplier Enrollment, Ordering and Referring, and
          Documentation Requirements; and Changes in Provider Agreements (RIN: 0938-
          AQ01). We received the rule on April 25, 2012. It was published in the Federal
          Register as a final rule on April 27, 2012, with a stated effective date of June 26,
          2012. 77 Fed. Reg. 25,284.
