      DIGEST:.
1.  Completion after bidlope;ng of various stanr'k-d representations
    and certifications is proper since 'hey do n- elate to bid re-
    aponsiveness and failure to submit them with bid may be waived
    as minor informality.
2.  Failure of low bidder to list ambulances and equipi eA it would
    use in perfoniLng ambulazine service' ontract did hot render
    low bid nonresponsive as omission relates to respionAthility
    of bidder rather than to responsiveness of bid, since procure-
    ment was for furniuhing of services and not for furistshing am-
    bial.4ces and equipment except a1s incident to furnishing services,
    and since bidder is legally obligated to furnish ambulances and
    equipment having acceptible minimum characteristics.
    The Veterans Administration (VA) has requested 4n advance
decision as to whether It can accept the apparnt low, bid for am-
bulance seririce under iivitation for bids (IFB).No. 619-10-77 issued
by the VA Hospitil. MoriLgbmry. Alabaima. The  low bidder, City
Ambulifrice of Alabama (City). iand the second low bidder, Haynes
AmbulinceServie'(Haynes),  lailed'to coihiplete various certifica-
tions and ep(esentations In thir bids. Both bidders failed to corm-
plie the  Equal Opportuidty and Affiliation and Identifjrig Data
provisions on Standard Form 3. iis well as the certifications in
the-Special Conditions of the IF. for Affirmativc Action Compliance
Program,  Minority Business Enterprise,  and Employment of
the Handicapped. In addition, City also left blank the Contingent
Fee representation on Standard Form 33.
   VA  has recorifmended that itk bepermitted to waive Haynes'
failure to complete the various :ertification and representations
as minor informalities. We are of the opinion that the. omissions
on the part oi both City and Hatneu may be properly ivaived as minor
Informalities or irregularities under section 1-2. 405 of th-* Federal
Procurement  Regulationa (19864 ed.).
   Our  Office has consistently held that completion of the subject
representatiens and certifications is not required to determi ine
whether a bid meets the requtrements of the specifications or other
solicitation provisionu and therefore does not affect responsiveness
