     SUBJECT: Revised Bankruptcy Forms Manual (INFORMATION)


            I am pleased to transmit the revised Bankruptcy Forms Manual containing the Official
     Bankruptcy Forms and selected procedural forms for use in the bankruptcy courts. The new
     manual also contains instructions for completing and using all of the forms. The Bankruptcy
     Forms Manual will be available electronically on both the J-Net and the federal judiciary's public
     Internet website within the next several months. This printed volume is for your reference.

            The Bankruptcy Judges Division has devoted major resources to this complete revision
     of the Bankruptcy Forms Manual, which was first published in 1988. Many forms have been
     updated, and all are current through December 1999. For example, discharge orders for chapter
     13 debtors, chapter 12 debtors, and joint debtors in chapter 7 cases have been revised to conform
     to the 1997 revision of Official Form 18, the chapter 7 discharge. For the first time, all of the
     Official Bankruptcy Forms prescribed by the Judicial Conference are available in one judiciary
     publication, and basic instructions to assist the general public also are included.

           The Bankruptcy Forms Manual is published as a service to both the courts and the public.
     Although courts now can generate many forms locally, Rule 9009 of the Federal Rules of
     Bankruptcy Procedure authorizes the Director of the Administrative Office to issue forms in
     addition to the Official Bankruptcy Forms, for use in bankruptcy cases. The procedural forms
     chosen for publication in the manual are those for which there are strong reasons for national
     uniformity; they also are those that the courts, the parties, and their attorneys are most likely to
     need. The forms and instructions may be copied freely, and clerks of court may make paper
     copies upon request for the usual charge of 50 cents per page. When posted electronically, the
     forms will be in portable document format (PDF) and can be downloaded to a personal computer.
