OCTOBER 26, 1942.-Committed to the Committee of the Whole House on the
state of the Union and ordered to be printed with illustrations
Mr. NICHOLS, Chairman of the Select Committee to Investigate Air
Accidents, submitted the following
REPORT
[Pursuant to H. Res. 125 and H. Res. 403]
CONCERNING AN ACCIDENT WHICH OCCURRED AT APPROXIMATELY
4:43 A. M. ON NOVEMBER 4, 1940, WHEN UNITED AIR LINES' TRIP
16 CRASHED WHILE IN THE PROCESS OF MAKING AN APPROACH
TO THE SALT LAKE CITY (UTAH) AIRPORT, RESULTING IN THE
DEATH OF SEVEN PASSENGERS AND THE THREE MEMBEi.C OF
THE CREW
I. CONDUCT OF INvEsTIGATION
Executive hearings to determine the cause of this accident were
held by your committee in Salt Lake City, Utah, on June 23, 24, 25,
26, and 27, 1941. Members of the committee present were Hon.
Jack Nichols (chairman), Hon. Richard M. Kleberg, and Hon. Carl
Hinshaw. Others present for the committee were Irving G. McCann,
counsel; Charles S. Greaves, clerk; and Jane Young, reporter. At
the conclusion of the hearings in Salt Lake City the chairman
instructed the committee's counsel to take the depositions of other
witnesses in Los Angeles, Calif., on July 2, 1941.
Before taking any testimony Mr. Nichols and Mr. Kleberg explained
to the witnesses the character and purpose of the investigations which
were being conducted by your committee. They stated that the
committee was conducting its hearings in executive sessions for the
purpose of securing all available evidence that would add to the safety
of air travel; that the purpose of the committee was not to punish
anyone; and that its objective was to conduct a serious and conscien-
tious search for all of the facts which would enable it to recommend
regulations or prepare legislation insuring greater safety to those who
travel by air. The witnesses were advised that if any or all of them
wished to make statements off of the record, they would be privileged
